The excellent benefits of Greenspring’s on-site medical center are unique among other Springfield senior health care facilities:

  • Personal doctor-patient relationships. Our doctors go the extra mile, taking time to get to know you personally and understand your needs.
  • Same-day appointments. You can often make an appointment for the same day you call, since our physicians exclusively see Greenspring residents.
  • Convenience for all. You’re still able to take advantage of the on-site medical center, even if you choose to keep your current doctor.

Rizwan Dar, M.D.

Rizwan Dar, M.D.

Medical Director

Dr. Dar received his medical degree from Punjab University in Pakistan and completed his residency in internal medicine at Illinois Masonic Medical Center in Chicago, Illinois. He is board-certified in nephrology and internal medicine and has a fellowship in nephrology from Hahnemann University Hospital in Philadelphia, Pennsylvania and a fellowship in special immunology from Jackson Memorial Hospital in Miami, Florida.  Dr. Dar has practiced in the Springfield, Virginia area for 15 years and served as medical director of Loehmann's walk-in clinic. He joined Greenspring in July 2017.

Kamal Sethi, M.D.

Dr. Sethi is board-certified in internal medicine and nephrology. She completed her residency and internship at Michael Reese Hospital in Chicago, Illinois. She was an attending physician at Howard University, Georgetown University Hospital, Sibley Hospital, Providence Hospital, Inova Fair Oaks Hospital, Inova Fairfax Hospital, Washington Hospital Center, and Specialty Hospital at Capitol Hill from 1996 to 2014. Dr. Sethi served as chief of clinical excellence at Providence Hospital from 2010 to 2013 and program director of internal medicine at Providence Hospital from 2010 to 2012. She joined Greenspring in May 2014.

Lakshmi Bhagavathula, M.D.

Dr. Bhagavathula is board-certified in internal medicine. She completed her residency in internal medicine at Prince George's Hospital Center, Maryland. Dr. Bhagavathula was in private practice from 2003 to 2006 in Fairfax, Virginia. She was an attending physician in Arlington, Virginia. She is experienced in geriatric care and worked in various retirement communities prior to joining Erickson Health Medical Group. She joined Greenspring in November 2014.

Annie Thomas, M.D., M.R.C.P.

Dr. Thomas received her bachelor of medicine and bachelor of surgery degree from the University of Ibadan, Nigeria, and her Membership of the Royal College of Physicians from the United Kingdom. She completed her residency in internal medicine at Wilson Memorial Regional Medical Center in Johnson City, New York. Dr. Thomas is board-certified in internal medicine. She joined Greenspring in January 2017.

Mildred Prah, D.P.M.

Dr. Prah received her bachelor's degree in biology from the University of Maryland Eastern Shore in Princess Anne, Maryland. She received her doctor of podiatric medicine degree from Temple University in Philadelphia, Pennsylvania. Dr. Prah joined Greenspring in September 2010.

Cavetta Green, N.P.

Ms. Green received her bachelor’s degree in nursing from the University of Arizona in Tucson, Arizona, and her master’s degree in nursing from George Washington University in Washington, D.C. She is a certified nurse practitioner. Ms. Green joined Greenspring in June 2017.

Kauser Kaleem, F.N.P.

Ms. Kaleem is a board-certified family nurse practitioner and holds a stroke certification from the National Institute of Health. She received her bachelor of medicine and bachelor of surgery degrees from Dow Medical College in Karachi, Pakistan. Ms. Kaleem joined Greenspring in June 2018.

Kathleen Taylor, L.C.S.W.

Ms. Taylor received her bachelor’s and master’s degrees in social work from Western Michigan University in Kalamazoo, Michigan. She is licensed in clinical social work as a mental health professional. Ms. Taylor joined Greenspring in August 2005.

Ashley Falcone, P.M.H.N.P.-B.C.

Ms. Falcone received both her bachelor’s degree and master’s degree in nursing from Wilkes University in Wilkes-Barre, Pennsylvania. She is a board-certified psychiatric-mental health nurse practitioner. Ms. Falcone joined Greenspring in August 2019.
